This form is designed for agencies, organizations, and community groups seeking support for events, programs, or initiatives that benefit children and families. By completing this form, you help us understand your needs and ensure that United Way resources are used to make the greatest impact in our community.
When submitting a request, please provide the following information:
- Number of children reached – How many children are expected to participate or benefit from the event.
- Number of families reached – Estimate of the total families who will benefit.
- Purpose of the event – Briefly describe the goal or mission of the event.
- Requested items – Specify the resources you are requesting:
- DPIL Books (include quantity)
- 211 informational materials
- Any other resources needed
- Community impact – Write a paragraph explaining how the event benefits the community and how United Way was recognized for its support.
- Photos – Upload photos that showcase the event or program, if available.
